Drug Request Forms

Secondary care only drug applications will be heard at the MTW Drugs, Therapeutics & Medicines Management Committee (DTMMC) - see below for submission deadlines and meeting dates

All other applications will be heard at the Kent & Medway Formulary Working Group (FWG) for recommendation to the Integrated Medicines Optimisation Committee (IMOC)

Request form for the Addition of a New Drug or Drug Use to the Interface Formulary                    Use this form if request is for a new addition to formulary for a group of patients, for either a licensed or unlicensed drug or use.

Request form for the One-off Use of a Non-Formulary Drug                                                                           Use this form if request is a one-off use for a single patient only, for either a licensed or unlicensed non-formulary drug.

Request form for a new Ready-to-Use product (Hospital Only).                                                                     Use this form for requesting a new ready to use product for a drug that is already listed on the formulary.
